We are a small GP surgery with a team of experienced staff.
We work to promote the benefits of continuity of care, and to ensure that your doctor knows you as an individual. There are benefits to you in being able to see the same GP for your appointments and to know that they are aware of you and your family’s situation.
For this reason we encourage that your whole household is registered with us to allow us to co-ordinate your care where this is beneficial.

We would advise to check as close to release time as possible, i.e. 8.00am or 12.00pm for same day appointments and 10.00am for advanced bookable.
You can also book a telephone call back from a doctor, medication review with the pharmacist or smear test with our nurse.
NHS 111 online in Lambeth
Patients in Lambeth can now access the same urgent medical advice from the 111 phoneline at 111.nhs.uk(link is external).
111 online helps people get urgent healthcare using their smartphone, tablet or computer. People answer questions about their symptoms and receive tailored advice on what to do next and where to go. If necessary, they will also be able to speak to a nurse, dentist or GP.
This new service provides a fast and convenient alternative to the phoneline and aims to help manage increasing demand on 111 telephone services. It uses the same clinical algorithms as the phone service, which have been designed by clinicians with expertise in the field of urgent care triage and are subject to internal and external clinical testing, and independent review.
The service has been developed in response to user feedback and insight from patients who have accessed other urgent and emergency care services.

Changes in prescribing of over the counter medication
Lambeth CCG no longer supports the routine prescribing of self-care medications for acute (short-term) illnesses and minor ailments and health supplements, malaria prevention medicines for travel and selected travel vaccinations.

Our online Prescription Service will be changing from 31st January 2018. From this date, all prescription requests will need to be made via the Patient Access service and prescription requests will no longer be accepted via e-mail. You will still be able to drop-off written prescription requests at the surgery if you prefer. to find out more, click here

When We are Closed
If you need urgent access to a GP over the weekend, when we are closed, then call 111 or SELDOC direct (020 8693 9066), and you will be assessed over the phone. If a face to face appointment is needed, you will be offered an appointment in an access hub, which may well be at Gracefield Gardens (but could be somewhere else in Lambeth)
You can access extended opening times at four specially selected GP Access Hubs in the borough. Our GP Access Hubs offer appointments seven days a week at the following times:
12pm–8pm Monday to Friday
8am-8pm Weekends and bank holidays

The Exchange Surgery pledges to improve Patient Safety
The Exchange Surgery announces it has pledged to improve patient safety as part of the national campaign, Sign up to Safety.
Sign up to Safety is a national patient safety campaign to help the NHS in England build a safer NHS and address the problem of unsafe care and avoidable harm.
So far, over 200 organisations have pledged support and are committed to improve the safety of healthcare through personalised Safety Improvement Plans.
These outline the actions that will be taken in response to five Sign up to Safety pledges.

We apologise to any patients requiring the Hepatitis B vaccine for their travels. Global supplies are very limited and the shortage may last until early 2018. To ensure that stock is available for those individuals at highest and most immediate risk of exposure to hepatitis B, Public Health England has developed temporary recommendations to support clinicians undertaking an individual risk assessment.
Public Health England and Department of Health have been working with both vaccine manufacturers to institute ordering restrictions according to customer type. It has been agreed with the BMA’s General Practice Committee that general practice will not be able to order any adult hepatitis B vaccine stock until further notice.
