What is a Self-Referral?
A Self-Referral offers you access to a specialist without the need to see your GP first. A Self-referral may also reduce your wait to access an expert opinion, advice and treatment of your condition and puts you in control of your care.

- Your referral will be reviewed within two working days by a specialist physiotherapist.

The Minor Eye Conditions Scheme allows patients across Lambeth to access NHS treatment for a range of eye conditions. Instead of having to visit hospital for treatment, you can now be seen by a highly trained optometrist based in a local optician's practice. The aim of the scheme is to allow all patients to be able to access treatment quickly, near where you live and work. For more information and list of local opticians see here

Diabetes Prevention Programme – Healthier You
If you are at risk of Type 2 diabetes, you are able to register for the Healthier You: NHS Diabetes Prevention Programme. To be able to register online you will need the following information:
- Your NHS number
- Your latest HbA1c or FPG reading, including the date
- Your GP surgery name
These are required fields, and referrals without this information cannot be processed. To be eligible for this programme your HbA1c must be between 42-47.9 mmol/mol or Fasting Plasma Glucose between 5.5-6.9 mmols/l and dated within the last 12 months

Award winning self-referral community falls prevention classes -Connect - News for GPs and primary care colleagues - July 2016
Service users (or friends and family) are encouraged to self-refer through our telephone helpline (020 3049 5424, Monday to Friday, 9-5pm). We also accept healthcare referrals via email gst-tr.SpecialistAdultReferrals@nhs.net.
Through our telephone triage process, service users are directed to the most appropriate service for them which include community and other exercise classes, Falls Clinic, and 1:1 interventions.
Classes are run for 1 hour per week over 30 weeks, and are delivered by trained falls prevention instructors. There are currently 20 classes across Lambeth and Southwark and we are looking to expand this further.
